MRG to Patient
MRG to prior Patient. FHIR R4 targets: Patient. See the v2-to-FHIR IG.
Prior identifiers become a second Patient with active false. The survivor links replaces, and the prior Patient links replaced-by, so a client holding the old MRN can find the survivor. A08 / ADT_A01 does not admit MRG; we still map it so the retired MRN is not left live.
