MSH to MessageHeader. FHIR R4 targets: Bundle, MessageHeader, Provenance. See the v2-to-FHIR IG.
eventCoding is taken from MSH-9. MSH-7 becomes Bundle.timestamp, not MessageHeader.eventTiming (that element is not in R4).
