PID to Patient
PID to Patient. FHIR R4 targets: Patient, Account. See the v2-to-FHIR IG.
PID-2 and PID-4 were withdrawn after v2.7. Real senders still populate them, so they become Patient.identifier (External ID / Alternate Patient ID). PID-3 remains the official identifier list. A time on PID-7 becomes patient-birthTime and birthDate stays the date. A missing PID-7 offset is taken from MSH-7 without shifting the clock. That offset is the sender's zone when the message was sent and is not checked against the birth date. If neither field has an offset, patient-birthTime is not emitted. Bare CX.4 takes Identifier.system from MSH-4. CX.5 MC uses the AU Base Medicare URI.
